Over approximately two years, Diabetes Digital Coach will enrol people in the West of England with type one and type two diabetes, providing them the opportunity to better self-manage their condition using a specially selected suite of integrated digital tools.
Enabling individuals to ‘do the right thing at the right time’ the Diabetes Digital Coach platform will be launched in 2017 and will also encourage more timely and appropriate interventions from peers, healthcare professionals, carers and social networks.
How will Diabetes Digital Coach work?
Diabetes Digital Coach will offer an online service for people with diabetes, available via PC, smart phone and tablet.
People simply sign in, answer some questions about their health, lifestyle and how they currently manage their diabetes. The coach then directs them to the best digital tool to suit their individual needs.
The service offers a range of five different digital tools. These provide varying degrees of support, covering health and wellbeing, structured education, dietitian support, optimising physical activity, and insulin management.
Data comes from users via their smart phones, tablets, glucometers and wearable tech. [readmore]
